Investigation Summary
At approximately 2:30 p.m. on September 13, 1997, Employee #1 was part of a four-person crew using a forklift to attach a trailer to a truck in a yard across from an auto repair shop. During the operation, a chain slipped and a section of the trailer fell from the forklift onto Employee #1's right foot, amputating his big toe. The trailer had not been properly secured with the chain and slings.
