EMPLOYEE INJURED WHEN COLLISION CRUSHES FOOT

At approximately 7:30 a.m. on November 30, 1997, Employee #1, an industrial fork truck battery changer, was driving a Crown Stand-Up Industrial Fork Truck Model RR3040-45-S, Hughes Unit Number 221. Employee #1 was returning the stand-up to its assigned parking spot after changing the battery. He was driving south out of the battery area with the forks trailing and was about to turn right when he saw a coworker approaching from that direction. When the coworker saw Employee #1, he stopped the industrial fork truck he was driving. Employee #1 stated that he had thought about turning left but before he could, the stand-up crashed into a parked pallet jack. Just before Employee #1 crashed into the pallet jack, he placed his foot outside the running lines of the stand-up. Employee #1's right foot was crushed between the stand-up he was driving and the parked pallet jack. Employee #1 was hospitalized and his foot was later amputated.
