Investigation Summary
Employee #1, a plasterer, was working from a scaffold platform approximately 15 ft above the ground, putting a sand finish coat on the side of a structure. The scaffold platform was 10 in. from the side of the structure. The top of an open area on the side of the structure was 19 in. above the scaffold platform. Employee #1 was applying the sand finish to the flat underside of the opening when he fell through the opening to the concrete. He sustained fractures of the right elbow.
