Investigation Summary
Employee #1 was cleaning some rollers on a machine used to load and package Ritz crackers. She was using an ON/OFF device that was connected to a receptacle on the machine to jog it. Employee #1 reached into the rollers with her left hand while the machine was running. Her little finger was amputated at mid-point when it became caught in the chain drive between a roller and the machine's frame.
