Employee #1, a painter, was working from a ladder that had been set on a piece of carpeting over a marble floor. The ladder slipped out and Employee #1 fell approximately 18 ft, fracturing his wrist. The ladder had proper feet, but it been set on a piece of unsecured carpeting.
