Employee's Finger Amputated in Saw Blade

Employee #1, a trained saw operator, had been asked to assist another operator in changing the blade of a panel saw. When he arrived at the saw, the maintenance door was open, so he began to remove the saw blade. As Employee #1 was reaching in to remove the old saw blade, a third employee who was unaware of the servicing activity going on, turned the saw on. Employee #1 failed to use lock out/tag out procedures. Employee #1's middle finger was amputated and he sustained lacerations to the second, fourth, and fifth digits of his left hand. Employee #1 was hospitalized for his injuries.
