Investigation Summary
Employee #1 was being trained to work on a company-built prototype machine which was used to clamp together muffler parts by pneumatic compression. When he activated the air supply, the top of the pneumatic cylinder caught his left index finger between it and a metal bar, resulting in an amputation of the finger tip. He was hospitalized.
