Finger amputated when caught in machine

An employee was changing the setup of a wire-stretching machine. While replacing a sizing block, he inadvertently stepped on the covered button which controlled the operation of the machine. Since no lockout/tagout procedure had been used, the employee suffered an amputation to his right index finger when the machine was activated.
