Investigation Summary

Investigation Nr: 201141272
Event: 11/30/1999
Employee Killed in Fall From Bundled Roof Trusses

Employee #1, a master carpenter, was working on a religious temple building site. A bundle of trusses had been delivered and were standing upright on a relatively level asphalt driveway. The trusses were being held upright by two straps attached to a crane. Employee #1 walked up the 32 inch wide bundle to a height of approximately 10 feet. Intending to have the truss bundle laid down flat on the ground, he then released both straps attached to the crane. After a few minutes, and before he could reattach one strap, the bundle began to fall over. Witnesses were unclear as to whether Employee #1 then attempted to jump off the bundle to the side in the opposite direction the bundle was falling, or if he merely fell in an off-balance manner. As he fell, he struck his head on the truss lower cross beam, and was killed.
