Employee #1 was checking whether an obstructed light sensor had caused a machine to stop running. He was kneeling to examine the sensor when the machine resumed functioning, amputating the tip of his left index finger. Employee #1 later stated that he was sure he had not placed his hand in the danger zone while he was kneeling.
