Investigation Summary
At approximately 4:30 p.m. on August 8, 2003, Employee #1 was told to unload palletized empty wooden barrels from the bed of a pickup truck Employee #1 was to unload the pallets from the truck with the help of a Toyota fork lift truck (Number P27, capacity 4500pounds, Model Number 5FGC25, Serial Number 75438) and then load the pallets on racks inside the warehouse. Employee #1 brought the fork lift truck onto the ramp near the pick up truck and shut off the power of the truck. The fork of the lift truck faced the open end of the pick up truck. Employee #1 dismounted from the lift truck without setting the brake and he did not block the wheels. As Employee #1 stood between the fork lift truck and the pick up truck, the lift truck started moving down the incline towards him. Employee #1 quickly jumped on the bed of the pick up truck with his two legs dangling as he sat on the pick up truck bed. The upper part of the lift truck came and struck Employee #1's left foot crushing it and causing multiple fractures. Employee #1 was hospitalized.
