Investigation Summary
On March 24, 2003, Employee #1 and a coworker (an apprentice ironworker) were working in a group on the third level of a six story steel frame building. After the morning safety meeting the coworker came on the third floor. At the time of the accident the steel frame and decking operations were in progress. The second floor decking was complete and third floor decking was in progress. There was one 5-ft by 8-ft floor opening near the northwest corner of the building. About 20 ft from this opening and towards the edge of the building on the north side, there was another opening of the same size. The openings were to be used to insert 'floats' (for use by welders) just below the floor level. When the coworker walked up to the 3rd level he saw Employee #1 standing midway in between the two openings. The coworker went to the second opening and was waiting there for Employee #1 so they could insert the float in the second opening. The coworker looked back and did not see Employee #1. Employee #1 had fallen through the first opening approximately 15 ft to the steel deck. Employee #1 suffered a dislocated left knee and torn ligament. Employee #1 was hospitalized for four days.
