Employee's finger amputated when caught in machine

At approximately 9:35 a.m. on October 22, 2002, Employee #1, a maintenance mechanic of St. Hart Container, was working on the 301 Emba Flexo Folder Gluer machine. The machine was 20 years old and worked in two shifts. Earlier in the morning a coworker, a machine operator, pointed out that the counter photo eye was not mounted properly because one of the screws was missing. With the help of a new screw, Employee #1 mounted and adjusted the photo eye. He then asked the coworker to start the machine to test the counter, which was found not to work properly. While working on the photo eye, Employee #1 observed that the chain of the sprockets of the counter and ejector index wheel drive shaft was off. Employee #1 talked to the coworker and it was decided that Employee #1 would fix the problem by putting the chain back in position and that the counter would not run the machine till then. Employee #1 laid on his back on the floor and slid under the counter ejector so that the chain was accessible. The coworker watched Employee #1 in this position. Employee #1 repositioned the chain onto the sprockets and then noticed that the chain was pretty loose. He pinched the chain between his thumb and the second and third fingers of his left hand and wanted to apply tension to the chain. Just as Employee #1 touched the chain, this section of the machine was activated by the coworker. Employee #1's left hand ring finger was amputated and the middle finger tip was fractured. Employee #1 was treated at the St. Jude Hospital in Fullerton, California.
