Investigation Summary
On October 13, 1994, Employee #1, a carpenter, was finishing a job for Wayne Chalk Painting. He had been temporarily loaned to them by Maranatha Builders, his regular employer. Employee #1 was climbing a metal extension ladder to a balcony area in order to mask the windows above the balcony in preparation for painting them. The ladder, which had not been secured, was positioned on a hard, smooth driveway surface scattered with pine needles. The bottom of the ladder slipped out and Employee #1 fell to the ground, landing on the ladder and fracturing his neck and separating his spinal cord. The local fire department brought immediate first aid to the site, and Employee #1 was transported by ambulance to Barton Hospital in South Lake Tahoe. He was then sent by Life Flight to Washoe Hospital in Reno, Nevada for treatment. On October 17, 1994, Employee #1's life support systems were removed and he died.
