Investigation Summary
Employee #1 was in a cutting room cutting stock for rubber stamps. She was using the table saw which has a plexiglass guard that extends from the top of the guide to the saw's blade. When the guide is extended back toward the operator, the top and rear portions of the blade are exposed. Employee #1 does not have a clear memory of exactly what happened, but her right hand came in contact with an unguarded portion of the saw blade. One finger was severed, three others were injured.
