Investigation Summary
The employer operates a sawmill at the location of the accident. Employee #1 was working as a grader sorting lumber at the infeed to the trimmer saws. The upper "Lumber Systems Inc." trimmer saw, Serial Number 111010R was in normal operation when it propelled a 6-foot-1-inch-long by 2-inch by 4-inch board out the infeed side of the machine with great force. The board struck Employee #1 on his chest above the heart causing a bruised heart, a collapsed lung and broken ribs in six places. Employee #1 was doing his normal job and did not know that what hit him. The board had a saw kerf along most of its length indicating that the saw drove the board out of the machine. The employer was cited for not guarding the trimmer saw in front by using a steel framework with a rigid metal screen.
