EMPLOYEE'S FINGER AMPUTATED BY FALLING TRACTOR AXLE

Employee #1 was in the process of changing the front wheel on a John Deere tractor, Serial Number 350485L. The tractor had been jacked up using the hydraulic bucket on the front end of the tractor. The front axle was blocked and the front wheel was removed. When Employee #1 went to put the new wheel on he found the axle wasn't high enough. Employee #1 had another employee use the bucket to raise the axle. While in the process of putting another block under the axle, the axle came down on Employee #1's finger, amputating it above the first knuckle.
