Investigation Summary
After Employee #1 finished dumping asphalt onto a roadway, he pulled his truck forward about 30 to 40 ft to clean out the truck bed. He positioned himself between the tailgate and truck bed and was cleaning out asphalt when a road grader hit the tailgate and crushed him. Employee #1 was taken to Madera Hospital for treatment of a crushed right arm and abdomen.
