Investigation Summary
At approximately 1:30 p.m. on March 30, 1998, Employee #1 was repairing a leak around a vent pipe on a roof of a two-story private residence. Employee #1 placed his portable extension ladder which he believed to be 20 in. on the ground level to gain access to the first story roof over the garage, which measured 8 ft 4 in. at its lowest point to 10 ft against the second story wall. He climbed the ladder, pulled it up behind him, and placed it on the first story roof to gain access to second story roof. The ladder slipped out from underneath him, causing Employee #1 to fall from the top of the ladder to the first story roof. The distance from first story roof to second roof could not be measured; however, photographs were taken. Employee #1 received compound fractures to his right ankle and lower leg which required hospitalization for 12 days.
