Investigation Summary
On September 25, 2010 Employee #1 sustained a fatal injury to himself. Employee #1 was brought in to trouble shoot a hydraulic system problem on a bankout machine. The repair was made and the safety prop was removed. For some unknown reason, Employee #1 started looking under the raised bed and it came down quietly on his head causing his fatal injury.
