Investigation Summary
On February 3, 2006, Employee #1 was setting up a guide string on the corner of a new block wall to lay a course of block. Employee #1 bent over to reach for something on the work surface and his face scrapped the pin projection was attached to the string and the pin popped out striking his left eye causing the serious injury. Employee #1 was hospitalized.
