Investigation Summary
At 10:15 p.m. on January 12, 1996, Employee #1 was climbing a metal ladder in Warehouse #1 to adjust a Strap Pak machine that had misfed. The heads of the machine were on a platform about 5 ft from the floor. Employee #1 later stated that his left foot either slipped or missed the fourth rung of the five-rung ladder that provided access to the platform. He fell approximately 4 ft, fracturing his left leg. Employee #1 also stated that he had been trained in the company's safety program.
