Employee's hands injured when caught in conveyor

At 7:30 a.m. on July 10, 1995, Employee #1, a trained and experienced worker at George Brothers, Inc., was cleaning a 60 in. wide conveyor belt. Rather than stop and start the machine, he tried to scrape the hugger/take-up roller and conveyor belt with a putty knife. Because he was working alone, he did not lock out the conveyor. He had, however, attached a lockout tag to the control switch, with the switch in hand mode. He then climbed under the belt and started scraping the roller with the conveyor belt running. The belt carried the scraper and Employee #1's right hand and lower arm over and into the nip point. In his attempts to extricate the scraper and his right lower arm, his left hand and lower arm also became caught in the take-up roller. Employee #1 was transported by ambulance to the hospital, where he was admitted and underwent surgery.
