Investigation Summary
Employee #1 was preparing to load bins of raisins from a metal rolling rack onto a container truck. He was placing them on a dumper platform while the truck driver backed up to the loading ramp. The driver was not satisfied with the truck's position and pulled forward a few feet, intending to back in again. Employee #1 was already on the platform and was walking with his back to the truck, pulling the metal rack; he was not paying attention to the truck and did not realize that it had pulled forward. Employee #1 stepped off the ramp and fell 55 in. to the ground. The metal roller fell on top of him, landing on his chest. Employee #1 underwent surgery for chest injuries and was able to return to work.
