Investigation Summary
Employee #1 was 18 ft high, tying off cables during construction of a metal building. Two coworkers had been elevated in a work basket to the same level and had exited. Employee #1 got into the basket to be taken back down to the ground. The forks of the lift were not in the pockets of the platform and the basket was not tied to the mast. When the load shifted, Employee #1 fell 18 ft to the ground. Employee #1 was hospitalized with multiple fractures.
