Investigation Summary

Investigation Nr: 170694368
Event: 12/10/1993
Employee's finger cut as he jostles fertilizer in carrier

Employee #1, a field hand, was unloading fertilizer onto a carrier, which transported it to a hopper that dumped the fertilizer into storage bins. Employee #1 later told the owner and his supervisor that he thought the product was slowing down and decided to poke or jostle the product to move it more quickly. His finger was lacerated. Although he had remembered that he had been warned many times about shutting off the machine before cleaning or checking, he ignored the warning. His push stick was in his hand, and the ON/OFF switch was 4 in. from the trap door (clean out).
