Investigation Summary
Employee #1 was cleaning roofing debris from a roof by dropping the debris into a forklift box. There were several pieces of insulation lying on top of two 4 by 8 by 3/4 plywood pieces that were covering a 6 by 6 roof opening, creating a weak seam in the area where the two pieces met. When Employee #1 stepped onto the plywood to retrieve trash, the plywood slipped and he fell approximately 20 ft, hitting a tank. Employee #1 bounced off the tank and fell an additional 10 ft. He sustained serious head and facial injuries.
