Employee's foot amputated by falling trailer fame

An 8,000 lb fiberglass tank was being placed on a trailer frame by an overhead crane. Employee #1 had placed lock pins on both sides of the frame legs. The male end of the tank was to fit into the opening of the spinner/rotator and then rest on the trailer. As it was being slipped in, the jarring from inserting the shaft caused the trailer frame to move forward approximately 2 to 3 inches. An improperly placed lock pin slipped out, allowing the frame to fall approximately 6 in. Employee #1's foot, which was under the frame, was amputated.
