Employee Falls from Ladder and Later Dies

Employee #1 and his supervisor were working inside a wood framed garage that had the walls standing and the trusses were yet to be installed. Employee #1 and his supervisor were measuring the distance on the top of the south wall for the top plate that was needed. The top plate is the 2-in. by 4-in. piece of lumber that over laps the walls tying them together. The south wall had two A-frame metal ladders next to the inside of the wall with a wooden plank sitting on the sixth rung of each ladder. Employee #1 was standing on the fifth rung at a height of approximately 5 ft. The plank was 13-ft, 6-in. long. Employee #1 was on the east ladder on the rung just below the plank and the supervisor was standing on the west ladder on the top rung above the plank. The supervisor was higher up on his ladder as he was placing the end of the tape measure to the end of the wall. Employee #1 was holding the metal tape measure, had finished measuring and was starting to come down the ladder when he fell to the ground striking his head on the concrete floor below. Employee #1's injuries were fatal and he died later that day.
