Investigation Summary
On October 17, 2006, Employee #1 was working for Sierra Pacific Industries, which owned and operated a saw mill in Quincy, CA. He was the operator of a horizontal resaw. He was positioned approximately 30 to 40 feet away at the resaw's control station, when he observed that a small board was jammed in the conveyor roller area just in front of the energized saw blade. The board was a 2-by-4 some four feet in length. He walked over to the front of the horizontal band saw, reached into the hazardous area at the point of operation, and grabbed the jammed board to remove it from the conveyor. The board was jammed approximately 6 inches from the blade. The saw was still energized, and the blade was moving, when Employee #1 attempted to remove the board from in front of the saw. When Employee #1 grabbed the board, it came up and hit the blade, pulling his right hand into the blade. The saw cut the thumb on Employee #1's right hand completely through the bone between the first and second joint, amputating it. Employee #1 was taken to the University of California, San Francisco (UCSF) Medical Center Hospital. He was hospitalized for six days for surgery and treatment.
