Investigation Summary
Employee #1 was a laborer working at a saw mill in the area of the pony conveyor. Employee #1's job was to perform clean-up and contact mill-rights when mechanical problems arose. On the day of the accident Employee #1 was near the pony conveyor when the chain broke and then tangled up stopping the conveyor. Employee #1 then shut off the conveyor and removed the guard. After removing the guard Employee #1 used his hands to attempt to free the chain conveyor. As Employee #1 was putting pressure on the belt he lost his footing and fell jamming his left hand between the belt and the pulley to the motor for the pony conveyor. This crushed Employee #1's middle and ring finger. When Employee #1 pulled his hand out he removed his glove and saw bones exposed. Employee #1 was taken to the hospital. Employee #1 suffered the amputation of the first digit (bone loss) of the middle and ring fingers of his left hand. Employee #1 was treated and released.
