Investigation Summary
At approximately 10:00 a.m. on February 15, 1995, Employee #1 was attempting to surface a board using a jointer saw. Although he was using the push stick that was provided, the board started "chattering," then kicked out. The push stick came loose from his hand, allowing his left hand to contact the blade. Employee #1 was taken to the infirmary, treated for an amputation of the left index fingertip, and released.
