Investigation Summary
The employer, the California Department of Water Resources, operates the state water project at the Banks Pumping Plant, unit 4. Employee #1 was on a stepladder repairing a leak on an 8 in. recirculating valve at the bottom level of the plant. After replacing the follower, he checked for leaks by opening the valve on the upper right side. The valve bonnet blew out and hit Employee #1, who sustained a fractured shoulder and head/face injuries. Apparently he either had not replaced the bolts that held the bonnet or did not realize that full head pressure (117 psi) was behind the valve on the right side.
