Investigation Summary

Investigation Nr: 170197487
Event: 01/23/1995
Employee injured when pinned between loader and forklift

On January 23, 1995, Employee #1, who works for a recycling facility, drove his forklift into the plastic pit area and parked behind a front end loader. As Employee #1 was preparing to dump the contents of a bin that was on the lift's forks, the loader operator, who was apparently unaware of Employee #1's position, backed up after loading a hopper. The loader's left rear tire pinned Employee #1 against the forklift. He was taken to Brookside Hospital for treatment of a fractured hip and separated pelvis.
