Investigation Summary
Employee #1, a finisher for an employer who fabricates steel water pipe fittings, was sandblasting the end of a 30 ft long, 7 ft 2 in. diameter pipe that had been set up earlier on wood blocks. The pipe was about a foot above the ground and chocked on both sides. While Employee #1 was sandblasting the pipe, it apparently moved and rolled over him. He was taken to Davis Medical Center with a skull fracture,
