Employee injured while avoiding a falling segment of steel

During work at the north side of the A1 cut and cover excavation of the City and County of San Francisco Muni Metro Turn Around project, the operator of a P&H truck crane (model 9125TC, serial #42151, with a rated capacity of 140 tons) lifted a load of six long segments and one short segment of tunnel steel liner. When it was halfway across the excavation, one of the two nylon slings holding the load loosened and the segments fell. As they fell, they hit the steel spreaders, making noise and alerting the workers below. Employee #1, a motorman at the track decking who was installing gaskets into the bolts for the tunnel liner, attempted to run away and take cover from the falling segments. He sustained minor cuts and abrasions to both of his hands and his face and he sprained his right wrist. All the employees involved in this near-miss accident were employed by Tutor Saliba Perini, the prime contractor. The primary cause of this accident was failure to properly secure the wet nylon sling before it was used to basket-hitch the damped load.
