Investigation Summary
Employee #1 had worked as a crane operator for four weeks at the Islais Creek Transport/Storage Facilities project for Rodio Icos Joint Venture constructing the slurry wall of the sewage storage collection basin. October 21, 1994 was supposed to be his last day on this job, because the job was winding down, but he was told to come to work on October 24, 1994, for a few hours to get his paycheck. At approximately 7:00 a.m. on October 24, 1994, Employee #1 was informed that the mobile hydraulic crane (Grove RT-755) (serial #49225) had been used that morning by the prime contractor and that the crane was running off the sprocket in reverse. In an attempt to correct this problem, Employee #1 swung the crane boom over the side toward the rear to keep away from the other employees. He boomed out to let the cable all the way out so he could bring the cable in the correct way. During this procedure, the rig overturned on its right side. Employee #1 was wearing his seat belt and no employees were hurt. The following conditions were contributory causal factors to the accident: The boom extension to 59 ft 6 in. at a 34 degree angle and 51 ft radius was beyond the load chart capacity; the outriggers were not extended and set; the crane was not leveled; and the tires were not inflated to the manufacturer's recommended pressure.
