Welder's foot struck by safety clamp from crane

Employee #1 is a welder employed by Tutor-Saliba-Perini at the Muni Turn Back Project in San Francisco. On his second day on the job, he was helping the welder leadman and the crane operator separate the 1/2 section plenum cans at the fabrication area at Pier 30. Employee #1's right foot was struck by a gripper hook attached to the safety hook of the P&H Omega 40 crane by a rigging of shackle and cable sling. The gripper is a Safety Clamp, Inc., model VLC, serial #394620. This gripper, which weighs 49 pounds, dropped about 4 feet on top of Employee #1's foot after he unlatched the clamp from the pad eye of the plenum can. He suffered a laceration on his fifth and fourth toes. His fourth toe was amputated 12 days after the accident.
