Employee injured when struck by angle iron

Employee #1 was working with the conveyor maintenance crew at the crusher building of the quarry department in a cement plant operation. He was helping pull the counterweight of the 9CR12 crusher flywheel to facilitate its restarting. The crew was using a chain hoist with a choker attached to a concrete beam at one end and to the spoke of the flywheel on the other end. As Employee #1 was tightening the chain hoist, the lower choker was obstructed by the flywheel guard. When he released the hoist to re-rig as ordered by his supervisor, a 2 in. by 42 in., 12 1/2 lb piece of angle iron that was welded to the guard dislodged and fell 3 ft, striking his left shoulder and head. Employee #1 sustained a concussion, cervical strain, and shoulder contusion.
