Investigation Summary
On February 17, 1995, Employee #1, a cabinet maker for Quality Cabinet Shop, Inc., was cutting a 5/8 in. thick by 5 1/2 in. wide by 2 ft long piece of particle board on an unguarded Rockwell table saw when the fingers on his left hand were lacerated.
