Investigation Summary

Investigation Nr: 170354179
Event: 10/12/1994
Employee injured when pinned between dock and falling gates

On October 12, 1994, Employee #1, of Economy Fence, was at a U.S. Army defense depot loading 10 ft 6 in. by 10 ft 8 in. gates that weighed approximately 300 lb. The gates were stacked vertically and secured with #9 wire to 2 to 3 in. diameter poles. Apparently, the wire failed and seven gates tipped over onto Employee #1.
