Investigation Summary
At about 2:00 a.m. on April 23, 1994, an employee's arm was caught by a projecting bolt from a revolving powered take off driveline that was unguarded. The unguarded shaft was 2 inches in diameter by 6 inches long. The shaft powered the rear mounted pump for a 150-gallon spray tank that was run by a 1976 Kubota farm tractor.
