Employee's fingers crushed by forks of forklift
At approximately 9:30 a.m. on July 18, 1995, Employee #1 was remounting a forklift fork to a mounting bar. His right hand middle and index fingers were at the back of the fork when it was dropped into the mounting slot. His fingers were crushed and the tips were later amputated at the hospital.
