Investigation Summary
Employee #1 and coworkers were working about 38 ft above a concrete floor on the metal roof of a building. They were applying 6-ft-wide, 120-ft-long rolls of insulation from the side of the roof to a center beam, and then attaching formed metal sections that snapped together. Employee #1 was attaching one end of the insulation to a metal pylon next to the center roof beam. He normally drove two washered metal screws into each side of the insulation to hold it in place. He would then signal to two coworkers in an aerial lift at the building's edge. They would pull the insulation tight. This time, however, he drove in one screw, looked up, waved at the men in the basket, and then tried to drive the second screw. The men pulled the insulation and the tension pulled his left foot, which was resting on the edge of the roll. He lost his balance and fell to the concrete. He was killed instantly.
