Investigation Summary
Employee #1, a millwright, worked for Sierra Pacific Industries. While repairs were being made at the debarking area of the sawmill, he was welding on the debarker kickers, which push logs off of one conveyor and onto another. Employee #1 was using rubber O-rings to hold the toggle switch in the 'up' position for the kicker on the rig side. When the O-ring failed, the kicker closed, striking Employee #1 on the head and pinning him to the side of the conveyor. He was killed. Employee #1 had locked out the electrical panels for the conveyors, but he failed to lock out or shut off the air to the kickers.
