Investigation Summary

Employee #1 drove a water truck for a small logging operation, spraying water to keep the dust down on the roadway. His day usually began between 3:00 a.m. and 4:00 a.m. At approximately 5:30 a.m. on the day of the accident, it was still dark when he met a logging truck coming into the landing. The road was too narrow for them to pass each other. Employee #1 began backing up to a wide spot when his truck went over the road bank. He was thrown from the vehicle, sustaining fractured bones in his neck and back. The truck rolled 300 feet downhill; the engine was torn loose and found another 200 feet below the truck. Neither CAL/OSHA nor the California Highway Patrol found any violations.
