Investigation Summary
Employee #1 works for Sierra Pacific Industries, which operates a sawmill. The maintenance supervisor and a millwright were cleaning up chips in preparation for repairing a torn belt on a chipper overs belt conveyor. The tail pulley hinged guards were lifted to facilitate cleaning with extension tools. Employee #1 joined to help but was told to stay clear. The conveyor was jogged three times to get the torn belt in position. When Employee #1 saw the conveyor stop, he thought they were done and went to the tail pulley to brush some chips off. When the machine started back up, it pulled his arm in. The operation was stopped when they heard Employee #1 yell. The accident was caused by the hinged guards not being put back in place prior to operating the conveyor. Also, Employee #1 did not receive adequate instructions concerning the procedure to be used. The employer was cited for serious violations of 3999(b) for the tail pulley not being guarded and 3314(a) for the conveyor not being locked out.
