Investigation Summary
Employee #1, a truck driver for Farm Land Management Services, which grows harvest rice and hay, was to haul hay from the fields to storage sheds and barns. He was atop a load of hay preparing to tie the load so it could be hauled from the field. Employee #1 got too close to the edge of the load and a bale slipped out as he stepped on it. He fell to the ground and suffered injuries to his right leg and back.
