Investigation Summary
On December 5, 1995, Employee #1, of Strain Ranches, Inc., and his supervisor were inside the farm shop taking the elevator off a John Deere 75 cuber wagon. The supervisor left for a short time and Employee #1 continued to unbolt the elevator. With unbolting completed, Employee #1 got a forklift and put the forks under the sloping elevator in an attempt to pick it up. He began pulling at the lower end tail pulley and somehow (Employee #1 does not remember) his right ring and little fingers and a portion of his palm were pinched and amputated. University of California at Davis physicians were not able to re-attach the fingers. Employee #1 did not have the load secured to the forklift, only balanced on the forks, which caused the load to shift and pinch his fingers. He is a long-time employee of this ranch operation, which grows alfalfa and converts the alfalfa into cubes for animal feed.
