FALL FROM LADDER

A production supervisor for an olive processing firm was working near a 2.9-meter-high olive tank. He was apparently climbing an unsecured, 3-meter, straight fiberglass ladder to gain access to the tank when he fell. The back of the employee's head hit the concrete floor. He died of his injuries.
